Assessment and implementation strategy
Every three years, Wentworth-Douglass Hospital conducts a Community Health Needs Assessment (CHNA) and develops an implementation strategy to address the community's top health needs identified through the CHNA.
Based on a comprehensive data assessment and input from community members, the 2025 Community Health Needs Assessment identified nine significant health needs in our service area. These are (in alphabetical order):
- Access to care and services
- Chronic disease
- Mental health
- Nutrition, physical inactivity and obesity
- Social determinants of health
- Substance use disorders
The 2025 Community Health Needs Assessment was approved by the Board of Trustees on September 2, 2025.
Current implementation plan
An implementation strategy has been developed to address these health needs over the next several years. This plan includes:
- Financial assistance
- Expanding access to primary care and Behavioral Health services
- Care-Van transportation
- Educational events & health screenings
- Operating a community dental center
- Community partnerships
