- Mass General Brigham Home Care: Serving the greater Boston area.
- Cooley Dickinson VNA and Hospice: Serving Hampshire County and parts of Berkshire, Franklin, and Hampden Counties.
- Wentworth Homecare and Hospice: Based in southern New Hampshire, and serves Strafford (NH), Rockingham (NH), and York counties (ME).
For patients who live outside the service areas covered by the Mass General Brigham home care programs above, the Home Health Collaborative offers access to additional trusted home health providers. These partner agencies help fill geographic gaps and are selected based on quality, safety and patient-centered care standards.
What is the Home Health Collaborative?
For patients who live outside the service area for Mass General Brigham home care agencies, the Home Health Collaborative offers another high-quality option for care at home. This collaborative was established by Mass General Brigham to give all patients and families access to external agencies that offer the highest possible quality of care.
While we do not own or operate the agencies in the collaborative, each one has been vetted by a team of experts based on quality, patient satisfaction, and collaborative care practices.
Collaborative agencies provide personalized medical and rehabilitation services for a variety of needs. Services include:
- Skilled nursing
- Physical therapy
- Occupational therapy
- Speech therapy
- Social work
Home health care may include short-term support after a hospital or rehabilitation stay, as well as ongoing care for patients managing chronic conditions. Each care plan is customized to the unique needs of the patient to support a safe and effective recovery at home.
Agencies in the Home Health Collaborative also work closely with Mass General Brigham teams, sharing insights and best practices to continuously improve the quality of care across our system.
