Our data-driven, community-informed approach

We ask and we listen. We let community needs, data, and evidence guide our efforts toward healthier, more equitable communities. Through community partnerships, we collaborate to create a lifetime of health for all.

We develop evidence-based strategies to deliver care rooted in equity. Our innovative programs tackle the clinical and social factors that shape the communities we serve.

We work to improve health outcomes through high-quality programming, address and reduce health disparities, increase access to care for the communities we serve, expand innovative community programs, foster social and family support systems, improve knowledge of healthy habits and behaviors, and enhance educational and health career opportunities.

To ensure we meet these responsibilities, and the regulatory requirements for non-profit hospitals and health plans, we follow several planning and assessment processes. We regularly review and update these processes.

Community reports

Mass General Brigham Health Equity Strategic Plan

MassHealth requires an Executive Summary as part of its broader goal to make health equity a core accountability measure for hospitals and health plans operating in Massachusetts.

This overview summarizes our plans to identify and address health disparities in the communities we serve. It includes information such as the health gaps we’ve identified in areas such as pregnancy health and hypertension, the rationale for the inclusion of each, and the goals we’ve developed in each area.

Community needs assessments

Every three years, all Mass General Brigham hospitals conduct Community Health Needs Assessments (CHNAs) that identify and prioritize the most pressing health, social, and economic issues in our communities. Findings from these CHNAs directly inform our Community Health Improvement Plan.

Joint CHNA report

In 2025, Massachusetts General Hospital, Brigham and Women’s Hospital, and Brigham and Women’s Faulkner Hospital collaborated to produce our first joint CHNA report for the Boston and the North Suffolk regions.

We’re committed to reducing premature mortality and improving the health of the communities we serve by addressing both clinical and social determinants of health. The joint CHNA was conducted in collaboration with community partners, local health departments, and residents to identify key health priorities, disparities, and opportunities for action across our service areas.

The CHNA process reflects our ongoing dedication to community engagement, collaboration, and evidence-based interventions. By combining quantitative data with community input, we highlight the most pressing health challenges—such as chronic disease, behavioral health, and social and economic barriers—and guide the development of strategies that promote improved health metrics and sustainable impact.

Community health improvement plans

Findings from our community health needs assessments related to the most pressing social, economic, and health issues in our communities inform our Community Health Improvement Plans (CHIP). These outline systemwide strategies and investments that address priority health needs and social risks in partnership with our different communities.

Massachusetts General Hospital, Brigham and Women’s Hospital, and Brigham and Women’s Faulkner Hospital produced a joint CHIP plan for the Boston and North Suffolk regions based upon priorities identified in their joint CHNA report.

Community health innovation at Mass General Brigham

Through the Kraft Center for Community Health, and Mass General Brigham programs and partnerships, we’re breaking new ground—finding innovative ways to deliver care, address community needs, and build healthier communities.

From our health careers partnership with Edward M. Kennedy Academy for Health Careers and Bloomberg Philanthropies to the Kraft Center for Community Health, we’re investing in the healthcare workforce of the future and making better healthcare more accessible locally, regardless of a person’s zip code.
