Compassionate care for aging patients and caregivers

It can be challenging to manage your changing healthcare needs as you or someone you love gets older. Through PrimeCare, we offer personalized support to help older adults remain safe, independent, and well-supported at home and across care settings. Our elder care managers provide care management services, not primary care. We work with patients, families, and medical teams to identify needs, coordinate services, and connect caregivers with practical support. They can also serve as patient advocates.

Our PrimeCare managers perform evaluations and provide customized care plans to support the changing needs of aging patients, caregivers, and their family members.
Elder care services we offer

Our PrimeCare managers can:

  • Participate in meetings with healthcare providers
  • Communicate patient and caregiver concerns to providers
  • Explain treatment plans
  • Educate patients and caregivers about medications
  • Develop helpful systems for taking and refilling medications
  • Provide guidance for caring for patients with dementia
  • Schedule appointments with providers
  • Arrange for transportation
Comprehensive assessments for older adults

We do thorough assessments to help us determine each person’s needs and determine how we can best support their health, safety, and daily life. During this evaluation, a care manager reviews:

  • Cognitive status
  • Finances, legal arrangements, and insurance coverage
  • Home safety
  • Medical history, medications, and current healthcare providers
  • Nutrition and diet
  • Social and psychological well-being
In-home evaluations
We conduct in-home assessments to understand the patient’s living environment, safety risks, and daily challenges. Evaluations can also be performed in hospitals, rehabilitation centers, or other care settings.
Financial, legal, and insurance guidance

Meeting the rising cost of elder care services can be challenging. We can help our patients and their families maximize their financial resources and provide information about:

  • Community resources for transportation and home care agencies
  • Home care and equipment purchases
  • Insurance benefits and coverage
  • Legal concerns, including estate planning, healthcare proxies, advance directives, and powers of attorney
Caregiver support

Many caregivers are faced with making complex, emotional decisions for their loved ones. We can help caregivers by:

  • Advocating for patients when caregivers can’t be present
  • Conducting on-site visits to evaluate patient care
  • Arranging placement or transfers to facilities
  • Supporting communication between patients, caregivers, and providers
