Focus on geriatric primary care services
You receive care from primary care physicians with advanced training in geriatric medicine. Other care team members include:
- Cardiologists and psychiatrists who specialize in geriatrics
- Case managers
- Nurse practitioners
- Social workers
For older patients who need hospital-based care, we work closely with attending physicians and care teams. We can also help with transfers to a Mass General Brigham hospital if you first receive emergency care at a different location.
Because Mass General Brigham is a teaching hospital, you may also receive care from doctors who are pursuing additional training in geriatric or internal medicine.
After your hospital discharge, please make an appointment with us for follow-up care.
Our psychiatrist specializes in helping older patients who have anxiety, depression, memory problems, and other behavioral health concerns. They can also help you cope with a chronic disease or life-shortening illness.
As a member of your care team, the geriatric psychiatrist partners with your primary care provider. These experts work together to diagnose and manage behavioral health conditions unique to older patients. You have access to:
- Consultations and assessments
- Medications
- Talk therapy (psychotherapy)
Our geriatric social workers work closely with patients, loved ones, and caregivers. They can assist with:
- Care coordination, including home health services, assisted living, skilled nursing, and nursing home options
- Counseling to help patients cope with the challenges of aging
- Support to help caregivers manage stress
