Our patient- and family-centered approach is shaped by each person’s functional, emotional, and social needs. Specialists in geriatric medicine, geriatric psychiatry, rehabilitation medicine, nursing, and social work collaborate to create a personalized care plan. We coordinate care across the full continuum, including hospitalization, rehabilitation, nursing home care, and home care services.

Compassionate, personalized care

Specialized outpatient care before surgery
Our outpatient pre-operative program helps older adults with complex medical needs prepare for surgery. This program has helped decrease the length of patients’ hospital stays and readmission rates.
Collaborative inpatient support after surgery
Our Geriatrics Inpatient Fracture Trauma Service (GIFTS) specifically supports orthopedic trauma patients who must undergo emergency surgery. The program combines expertise from orthopedic specialists and geriatric doctors and has helped decrease patients' recovery and reduce post-operative complications.
Education and research leadership
Our clinicians teach, mentor, and collaborate to advance the field of aging care. Through ongoing research and innovation, we bring the latest evidence-based approaches directly to our patients.
