Prepare for your appointment
Preparing in advance for your appointment can help your visit go smoothly. Please plan to arrive early to complete registration and bring:
- A photo ID
- Your insurance card
- A list of prescription and over-the-counter medications and supplements
- Your medical history, including hospitalizations, procedures, and allergies
