Because choosing a Skilled Nursing Facility (SNF) is an important decision, we encourage patients and families to tour the facility to make sure it's the right fit for you.
The Skilled Nursing Facility Collaborative via Mass General Brigham was established to ensure that patients discharged from our hospitals to skilled nursing facilities have a choice of facilities offering the highest possible quality of care.
SNFs offer a wide range of rehabilitation programs, including services for musculoskeletal, neurological, complex medical, amputee, stroke, and pulmonary conditions. SNFs offer short-term care for patients recovering from an illness or injury, as well as long-term care for patients who require 24-hour care in a residential setting. SNFs offer a wide range of rehabilitation services as well as medical services that are customized to meet each patient’s needs.
A team of experts from Mass General Brigham selected the SNFs in the Collaborative based on quality measures and collaborative relationships with Mass General Brigham organizations and participation in Mass General Brigham’s Center for Population Health’s programs.

Skilled Nursing Facilities from Massachusetts were selected based on publicly available information, such as the 5-Star Quality Rating system used by the Centers for Medicare & Medicaid Services, as well as collaborative relationships with Mass General Brigham organizations and participation in Mass General Brigham Population Health post-acute transitional care management program.
The SNF Collaborative offers access to high-quality SNFs that care for a wide variety of patient needs. Our goal is to better coordinate your transition from the hospital to a skilled nursing facility and to enhance communication among your care providers. Better coordination of care means higher quality care and faster recovery times. Simply put, we want our patients to get better, faster.
In addition, the SNF facilities in the Collaborative work closely with Mass General Brigham teams to share information about successful programs and best practices with the goal of further improving care at our facilities.
Family and caregiver support is critical in helping with your recovery. We encourage family members, friends, or caregivers to be involved in the patient care plan and to ask questions. In all facilities, the patient’s clinical team, including a case manager and/or social worker, will work with you and your family to ensure a safe discharge or transition to the next level of care.
