Case Management

Our home health case management team assists patients and families with navigating the many services available to them in the home setting. 

What is Home Health Case Management?

At Mass General Brigham Home Care, home health case management is performed by the primary clinician assigned to a patient’s care with the goal of assisting patients and families with navigating the many services available to them in the home setting and helping them cope with complicated situations in the most effective way possible to achieve a better quality of life.

For example, our nurse case managers help home care patients and families with issues such as:

  • Identifying health care needs and forming a plan to meet those needs
  • Matching individuals’ needs to the right resources
  • Advocating on behalf of the patient and family to obtain needed services
  • Assessing the effectiveness of home care treatments and services
  • Suggesting changes to services if necessary to meet an individual’s changing needs
