Athletic Trainer Request Confirmation

Thank you for submitting an athletic training request!

A member of our team will reach out to you within 2 business days to provide more information about our program, send a template for your event requests, and begin to process necessary information for a service agreement. 

Please note:

• We are unable to begin athletic training services without a fully executed agreement.
• We will do everything we can to fulfill your requests; however, athletic training services cannot be guaranteed.
• Clients are required to ensure that an automated external defibrillator (AED) and helmet removal devices (when necessary) are present at all sporting events.
